The patient is sent for further investigation of bilateral fluidothorax and expanded mediastinum.

The ultrasonographic examination from the ventral left parasternal approach shows a large tumour mass pressing down on the large vessels. Biopsy with a true-cut needle carried out in real time; when the needle is closed artefacts from the closed needle can be observed. Histology confirmed diffuse large cell B-cell lymphoma stage IV.B.
